Your connection guide

Connect ChatGPT
to trulyform.

A few steps to bring your forms into the conversation. Start in ChatGPT, then sign in to trulyform when you’re asked.

A quick check

Before you start

  • Custom MCP access in your account or workspace. Managed workspaces may restrict connections.
  • A trulyform account with a verified email. If you’re new, create it when the connection asks you to sign in.
  • No trulyform API key or client secret. You approve access through your browser.

MCP is included on trulyform Free. Your assistant’s own plan requirements still apply.

Step 1

Add trulyform to ChatGPT

  1. In ChatGPT, open Settings → Security and login and enable Developer mode. Availability depends on your account and workspace policy.
  2. Open ChatGPT Plugins and select the plus button to add a connection.
  3. Name it trulyform. Add a description such as “Create and edit forms, publish drafts, and read responses.” Under Connection, enter the public MCP server URL below.

Select the text to copy it.

Create the connection and complete the OAuth sign-in when prompted. Choose OAuth if asked for an authentication method; trulyform does not use an API key for MCP.

After authorization, review the discovered tools. Start a new conversation and add the trulyform connection from the tools menu before trying the request below.

This guide uses a manual connection. It does not require a trulyform listing in ChatGPT’s directory. If Developer mode is unavailable, check your plan and your workspace’s policies.

Step 2

Sign in. Review. Approve.

  1. When your assistant opens trulyform in the browser, sign in. New here? Choose Sign up on the login page.
  2. If your email needs verification, follow the verification link in the same browser session. Then continue to the authorization screen.
  3. Check the assistant’s name, callback address, your account, and your organization. Choose Approve access if they are correct.
  4. Return to ChatGPT and finish any connection prompt. If you have approved this client before, consent may be skipped.

What you’re approving

Read access to forms and responses across your current organization, plus creating forms, editing drafts, and publishing. This includes teammates’ forms; there is no read-only or per-form selection. Requested data is shared with the assistant under its policies.

Step 3

Make sure you’re connected

Start a conversation with the trulyform connection enabled. Try a simple request that reads your forms:

Select the text to copy it.

Success means the assistant uses trulyform’s tools and returns your forms. An empty list is fine if you have no forms yet. A generic explanation of trulyform without a tool call does not confirm the connection.

Then make something yours.

Your first form prompt

Create a client intake form for my design studio. Ask for their name, email, project goals, budget, and timeline. Save it as a draft for me to review; do not publish it.

Select the text to copy it.

Open the draft in trulyform to review it. Publishing is a separate action: ask for it explicitly when you are ready. There is no new trulyform approval dialog for every publication.

A little help if you’re stuck

I cannot find the connection settings.

Update your assistant and check its current official instructions below. Custom MCP access can depend on your plan or workspace policy. Ask your workspace administrator if the controls are unavailable.

The server cannot be reached.

Copy the complete server URL, including /mcp, and check your connection. A hosted assistant needs a publicly reachable HTTPS endpoint; it cannot reach a localhost URL on your computer. Check whether your organization restricts external MCP servers.

Sign-in or email verification did not finish.

Complete verification in the browser session where you started the connection, then return to the assistant. If the session or authorization request expired, start authentication again from the assistant.

The connection exists, but no tools are available.

Finish OAuth sign-in and enable the connection for the current conversation. Refresh the client’s tool list or start a new conversation. Ask explicitly to use trulyform and list your forms. Adding a URL alone does not complete authorization.

I cancelled access, or it stopped working.

Run the client’s authentication flow again. You can also revoke the old connection in Settings → AI connections, then reconnect. Check that your trulyform email is verified and you still belong to an organization.

My form list is empty, or some responses are missing.

Check the account and organization you approved. A new account may have no forms yet. Ask your assistant to retrieve more pages if results are paginated. Free includes completed responses; Pro and Business also include partial responses.

How do I remove this connection?

In trulyform, open Settings → AI connections and choose Revoke access for this assistant. This blocks future access; it does not erase information already shared. Remove the saved server configuration from the assistant too if you no longer need it.
